Individual User Registration Form
Individuals
Each person who wishes to use DANSA services must register with the organisation to become a member. The registration form is very short but contains important information that we require in order to provide the best possible service to you. As well as details about yourself we are also asking for information on any disabilities or health problems that you may have. This ensures that we are fully prepared for all your requirements. The form also asks whether you use any equipment such as a wheelchair, walking stick, walking frame and whether you will require assistance in walking to and from the vehicle.
Finally, we also ask for the contact details of someone that we can contact in an emergency. Whilst the chances of you being involved in an accident whilst using DANSA are very small, it is important that we hold some emergency contact details just in case. Also, if you suffer from ill health having an emergency contact person will also enable us to contact a family member or friend should your health take a turn for the worse.
Organisations
Each organisation that wishes to use DANSA services or vehicles must register with the organisation to become a member. The registration form requires you to complete details about your organisation, what you require transport for and the requirements of the people using the vehicles (e.g. wheelchair users) and also an indication of whether you intend using your own volunteer driver or one of the DANSA drivers.
The information on the registration form will allow us to ensure that the people who are to drive the vehicles for your organisation are appropriately trained, that you are shown to how to safely secure wheelchairs in the vehicle and use the electronic tail lift, and so that we are able to make any special arrangements necessary for the people that you wish to use the vehicle (e.g. booster seats for children).
The form will also enable us to have a key contact within your organisation who are we are able to get in touch with to discuss bookings, journeys, costs, methods of payment etc.
